If you have watched any of the plastic surgery programs on television and have seen how a broken implant looks like, you will actually get scared. The doctor has to open the breast and remove the mess, especially in the case of silicone gel. It looks like a sticky gelatin on a carpet. Saline implants also have the risk of leaking and the FDA is keeping an eye on them as well. These cases are rare, but not unheard of.

The breast augmentation implants are very strong, and the possibility of rupture is low. Some high impact accidents can break them, but this is not the only cause.

Breast implants are considered by the FDA as medical devices. As such they have to pass the FDA standards, but this is no assurance of their being 100% safe. There is no such thing as 100% safe. 

Breast implants (saline or silicone) can rupture, harden, ripple, shift position, and change shape over time. 

Breast implants are not part of your body, so your body may react against them and cause infections, pain, loss of sensation, hardening, scar tissue, modifications of your breast tissue, modification of your breast shape, etc. 

Depending on the type of procedure, they can interfere with breast-feeding and breast cancer detection. For unknown reasons, the FDA reports that some women with breast implants, even when the surgery does not cut the milk ducts, are unable to breast-feed. There is simply insufficient milk production. 

The FDA is keeping an eye on the silicone implants safety and new evidence is being examined in the case of silicone implants. They may be reintroduced in the future.

In the US, the Food and Drug Administration has not approved breast augmentation surgery for patients less than 18 years of age.
